Case Management

Many of our programs offer case management services. The Screening Specialist on the 2nd floor of the Shelter (as well as the PATH Outreach worker on the 1st floor) can help determine programs that might be a good fit.

HopeWorks does not offer vouchers for gas.

Hope Village

Hope Village provides permanent supportive housing to people who have experienced chronic homelessness. As such, the building is not open to the public and cannot accept walk-ins. An extensive application, completed in conjunction with a service provider, is required. Openings in this 42-unit housing complex are rare. Speak to your service provider for more information.

Residents are individuals who meet the federal definition of “homelessness” or “precariously housed,” have a documented mental, emotional, or behavioral disorder, are able to complete daily living activities independently, have an income at or below 30% AMI, and meet other eligibility requirements, including a score of 13 or higher on a commonly-used “vulnerability index” (VI-SPDAT).

Residents sign a Residential Rental Agreement with the contracted property management company, Monarch Properties, Inc. The lease is a Standard Residential Lease Agreement found at other rental properties.

Residents can be evicted for not adhering to the Residential Lease Agreement.

Contact the Housing Division to find out more about our housing programs and what you may qualify for. Additionally, you can complete a coordinated assessment (a.k.a. VI-SPDAT) required for most housing programs at the Day Shelter with Shelter or Outreach Team staff.

Rental Assistance and Eviction Prevention

HopeWorks offers rental assistance and eviction prevention only to clients already enrolled in HopeWorks programs. Applicants must have a diagnosed and documented serious mental illness or co-occurring substance use disorder and currently be receiving behavioral health services to qualify for this program. Clients already receiving case management services at HopeWorks can ask about being referred.

Therapy and Psychiatric Services

Therapy: HopeWorks offers outpatient individual and group therapy for HopeWorks clients and the general public. We serve clients with Medicaid & Medicare and also offer a sliding scale for those with no insurance or with private insurance. Screening and enrollment for therapy services is completed with the Screening Specialist on the 2nd floor of the Shelter.

Psychiatric Services/Medication Management: Two nurse practitioners offer Psychiatric Medication Management. Staff support clients with a wide range of services, including counseling, mental health diagnoses, help with addiction, and prescribing medication. We serve clients with Medicaid & Medicare and also offer a sliding scale for those with no insurance or with private insurance.

Transportation

Currently, city buses are free in Albuquerque. Bus tickets beyond Albuquerque are not available on a regular basis. If you are in need of one due to extenuating circumstances, please talk to your case worker.

HopeWorks offers utilities assistance to clients housed through HopeWorks’ housing programs and for those engaged in behavioral health programming with any provider.
